Job Description

Seeking a dedicated Physical Therapist (PT) to join our home health team in Hudson or Essex County.

  • Provide in-home visits, develop care plans, and perform assessments.
  • Coordinate with care teams and educate patients and families.
  • Accurately document interventions and observations in electronic health records.

    • Qualifications include a current NJ PT license, reliable transportation, strong interpersonal skills, and electronic documentation proficiency. Bilingual skills and additional certifications are a plus. New licensees are encouraged to apply.
