Job Description

Seeking a Home Health Physical Therapist in Westchester, IL for full-time, dayshift work (40 hrs/week).

  • Position involves delivering evidence-based care, ensuring patient safety, and participating in quality improvement initiatives.
  • Collaborate with the care team, coordinate treatment plans, and maintain effective communication and documentation.
  • Build strong patient relationships, assess individual needs, and provide tailored education to promote self-care.
  • Requires IL licensure, at least 1 year of experience or clinical affiliation, and proficiency with home therapy equipment.
  • Must be able to travel extensively, lift up to 35 pounds, and work rotating weekends/holidays.
  • Offers a sign-on bonus up to $10,000, benefits including health insurance, retirement, continuing education, and mileage reimbursement.
