Job Description

Seeking a Home Health Physical Therapist in Elm Grove, WI for flexible, ongoing 40-hour weekly shifts.

  • Conduct patient evaluations, develop and update care plans, interpret physician referrals, assess responses, teach interventions, and communicate progress.
  • Manage caseloads, coordinate coverage, ensure regulatory compliance, perform joint visits with aides, teach treatment procedures, and assist with adaptive devices.
  • Participate in quality improvement, discharge planning, and mentor students.
  • Must have a valid Wisconsin or Illinois PT license, BLS certification, and at least 1 year of clinical experience.
  • Require strong clinical skills, knowledge of modalities, proficiency in Microsoft Office, and physical ability to lift up to 50 lbs, move in confined spaces, and travel for home visits.
