Home Health Physical Therapist – $43-64 per hour

Seeking a full-time Home Health Physical Therapist (PT) in Thornton, Illinois for 40 hours/week day shifts.

Provide patient-centered care focusing on quality and safety through evidence-based practices and documentation.

Collaborate with healthcare teams, participate in quality initiatives, and promote effective communication.

Build strong patient relationships, educate patients and families, and support self-care.

Demonstrate leadership, clinical decision-making, and adaptability to complex cases.

Travel extensively within the service area, working in patients’ homes and assisted living facilities.

Must hold a valid Illinois or Wisconsin PT license, BLS certification, and at least 1 year of clinical experience.

Physical ability to lift up to 35 pounds, assist with transfers, and work in varied environments required.
